Article Structure

The Texas DL 64 form serves as an essential tool for residents needing to update their address or replace a lost or damaged driver’s license or identification card. This application is applicable for both standard Texas driver licenses and commercial driver licenses, as well as identification cards. Completing the form requires providing personal details such as your name, date of birth, and social security number, ensuring that the Texas Department of Public Safety can accurately process your request. Alongside the basic information, applicants have the opportunity to indicate support for various causes, including organ donation and assistance for survivors of sexual assault, by contributing a small donation. For veterans, the form allows for the addition of a veteran designator, contingent upon providing proof of service. It’s important to note that submitting the DL 64 form incurs a $10 fee, which can be paid via check or money order. Once filled out, the form can be mailed to the designated address in Austin, Texas, or you can opt for a more convenient online address change process. This form is strictly for address changes or replacements; if other modifications are needed, such as changing personal information or updating your photo, a visit to a local driver’s license office is necessary.
